Any settlement is designed to compensate you for medical expenses and pain and suffering. If you receive medical care without paying up front and those providers are paid out of the settlement, you need to look at your settlement holistically. This means that you received medical care that has been paid for AND money in your pocket for pain and suffering. Isolating your settlement to only what you recieve in pocket is not the whole picture and will usually lead to disatisfaction.
